Job summary

Bay Aging seeks an Options Counselor to provide person-centered counseling to Veterans enrolled in the Veterans Directed Care program. You will assess needs, develop spending plans, and coordinate supports to help Veterans avoid nursing home placement.

The role includes home visits and office work with travel to Erie, PA and surrounding areas as needed. A Bachelor's degree in social work or a related field and at least three years of relevant experience are preferred.

Qualifications

• 3 or more years of experience working with seniors, disabled adults, human services or general case management field.

Responsibilities

• Home visits (50%): assess clients at home, develop a spending plan, assist with employer responsibilities, and conduct required contacts and reassessments.
• Office work (50%): follow up on referrals, maintain confidential information and records, and handle fiscal duties for VDC program.
• Travel coordination and reporting: manage travel and expense reports; monitor spending with the FMS provider and supervisor.
