Job summary

Rocky Mountain Human Services is seeking a Healthcare Navigator to connect Veterans with VA health care benefits and community services. You will coordinate care, educate Veterans, and work with a multidisciplinary team to ensure timely, equitable, veteran-centered care.

The role involves remote work with regular travel up to 50% and collaboration with HAV and RMHS teams to address veteran needs and improve outcomes.

Qualifications

• Master’s degree in counseling, social work, or a related field.
• Ability to pass required background checks.
• Ability to complete VA/RMHS required training.
• Two to five years in case management or care coordination (preferred).
• Knowledge of VA benefits, Medicaid/Medicare, and community resources.

Responsibilities

• Manage day-to-day responsibilities in a remote work setting.
• Conduct non-clinical assessments with care teams and Veterans.
• Develop care plans, monitor progress, and share updates with clinicians.
• Provide comprehensive case management and care coordination.
• Identify health education needs and provide tailored materials.
• Maintain documentation per contract requirements.
