Job summary

SSSEVA is seeking a Veterans Directed Care Options Counselor to provide person-centered guidance for veterans and caregivers in choosing home-based care. The role involves assessments, care planning, and ongoing support across remote work with travel up to 50% within the Durham service area.

A monthly in-person visit to the Norfolk headquarters is required, with collaboration to connect veterans to needed services and resources. Strong assessment and communication skills are essential.

Qualifications

• Bachelor’s degree in social work, human services, or a related field, or two years of community-based service experience.
• Strong knowledge of aging, disability, veteran, Medicaid/Medicare or related benefit systems preferred.
• Excellent listening and communication abilities; capable of empowering veterans and caregivers with a person-centered approach.
• Proficient with Excel and web-based systems; maintains confidentiality and HIPAA compliance.

Responsibilities

• Complete initial assessments and required documentation.
• Identify needs, preferences, and services with veterans and caregivers.
• Develop and monitor individualized care and spending plans.
• Explain responsibilities and available services to participants and caregivers.
• Provide guidance, training, and ongoing support as needed.
• Conduct regular participant contacts (monthly, quarterly, etc.).
• Follow up on referrals and ensure services are implemented correctly.
• Maintain confidential participant records in electronic databases.
• Monitor participant spending and complete required reports.
• Coordinate with healthcare providers and community partners to connect resources.
