Job summary

Volunteers of America Chesapeake & Carolinas is seeking a Case Manager to provide day-to-day support for participating veteran households across NC regional offices in Durham, Fayetteville, Rocky Mount, or Salisbury.

Responsibilities include outreach, eligibility screening, housing assessments, service coordination, and wraparound supports with VA and community partners. Travel within the Carolinas and telework capacity are involved as needed.

Qualifications

• Bachelor's degree in counseling psychology, rehabilitative counseling, or related human services field.
• Experience in homelessness prevention, rapid re-housing and transitional supportive services is preferred.
• Ability to coordinate with VA and community partners to deliver wraparound supports.

Responsibilities

• Coordinate outreach plans for the SSVF program, visiting VA facilities, shelters and community housing providers.
• Screen program participants for eligibility using VA-approved forms and verify information.
• Conduct housing assessments and develop comprehensive Family Services Plans.
• Provide service coordination between the program and community partners.
• Develop placement, stabilization and follow-up plans with clients and their families.
• Arrange housing support and wraparound services linking to VA and mainstream resources.
• Travel to housing sites to support clients and monitor progress.
• Communicate effectively with the service team while maintaining confidentiality.
• Complete required HMIS and other reporting; document services and outcomes.
