Qualifications

• Knowledge of disability and aging services systems and community resources.

Responsibilities

• Visit individuals in their homes and assess needs and preferences.
• Identify community options to meet identified needs and facilitate access.
• Coordinate with providers and caregivers to ensure services are obtained and followed up.
• Document case details timely and accurately using electronic records.
• Travel frequently to homes throughout the service area; manage scheduling by urgency and deadlines.
