Job summary

Oak Street Health is seeking a Clinical Call Center Triage Nurse to support patients remotely. You will handle medical, social, and psychological needs telephonically, documenting precisely and following refill policies, while maintaining high patient satisfaction.

The role requires an active RN license across OSH practice, strong communication, reliability, and the ability to work flexible shifts. A remote, home-based setup and US work authorization are expected.

Qualifications

• Active RN license in OSH practice across states
• Strong problem-solving and communication abilities
• Ability to work flexible shifts including evenings, weekends, and holidays
• US work authorization
• Proven reliability and attendance

Responsibilities

• Provide confident and professional service to OSH patients
• Follow current Prescription Refill policy
• Document accurately during and after calls
• Schedule appointments and arrange transportation
• Check the CareReq queue and ensure requests are complete
• Complete all tasks within scope and ability of the role
• Demonstrate initiative to meet departmental metrics
• Other duties as assigned
