Responsibilities

• Provide confident, professional service to Oak Street Health patients.
• Follow current Prescription Refill policy.
• Maintain clear, precise documentation during calls.
• Complete documentation for all patient requests prior to ending a call.
• Check CareReq queue and ensure CareReqs are complete.
• Schedule appointments for patients off-cadence or requesting appointments.
• Arrange transportation for clinic visits.
• Demonstrate reliability and meet departmental metrics.
