Job summary

Oak Street Health is seeking a Clinical Call Center Triage Nurse to extend care for patients by addressing medical, social, and psychological needs via telephone. This remote role supports clinic hours and requires coordination of appointments and transportation as needed.

The position emphasizes nursing licensure, patient-centered communication, and adherence to care policies while contributing to team metrics and quality outcomes.

Qualifications

• Active, non-probationary RN license in all OSH practice states.
• US work authorization.
• Ability to work flexible shifts (evenings, weekends, holidays).

Responsibilities

• Provide confident and professional service to OSH patients.
• Follow current Prescription Refill policy.
• Remain engaged with patients without background distractions.
• Clear and precise documentation while on a call.
• Complete documentation for all patient requests prior to ending a call.
• Check the CareReq queue and ensure CareReqs are complete.
• Complete all tasks within scope and ability of the role.
• Schedule appointments for patients off-cadence or requesting an appointment.
• Arrange transportation for clinic visits.
• Demonstrate reliability and meet departmental metrics.
