Job summary

Oak Street Health is seeking a Clinical Call Center Triage Nurse to extend care during and outside clinic hours, addressing patients' medical, social, and psychological needs telephonically. You will provide confident, professional service to OSH patients and document everything clearly on every call.

Responsibilities include following refill policies, scheduling appointments, arranging transportation, and ensuring CareReqs are complete while maintaining a patient‑centric approach and meeting

Qualifications

• Active, non-probationary RN license in OSH practice across states
• Proactive maintenance of licenses
• Ability to manage time and work autonomously
• Commitment to delivering unmatched patient experience on every call
• Completion of CE and licensure requirements

Responsibilities

• Provide confident and professional service to OSH patients
• Follow current Prescription Refill policy
• Remain engaged with patients without background distraction
• Clear and precise documentation while on a call
• Complete documentation for all patient requests prior to ending a call
• Check the CareReq queue and ensure CareReqs are complete
• Complete all tasks within scope and ability of the role
• Schedule appointments for patients off cadence or requesting an appointment
• Arrange transportation for clinic visits
• Aware of organizational goals and strive to meet departmental metrics
• Other duties as assigned
