Job summary

Oak Street Health is seeking a Clinical Call Center Triage Nurse to effectively extend care for patients telephonically, outside regular clinic hours. The role supports OSH patients with medical, social, and psychological needs and requires an active RN license in all OSH practice states and strong time-management in a remote setting.

Qualifications

• Active, non-probationary RN license in all OSH practice states.
• Proven reliability and strong time management in a fast-paced environment.
• Willingness to work flexible shifts including evenings, weekends and holidays.

Responsibilities

• Provide confident and professional service to OSH patients.
• Follow current Prescription Refill policy.
• Remain engaged with patients without background distraction.
• Clear and precise documentation while on a call.
• Complete documentation for all patient requests prior to ending a call.
• Check the CareReq queue and ensure CareReqs are complete.
• Complete all tasks within the scope and ability of the role.
• Schedule appointments for patients and arrange transportation for clinic visits.
• Aware of organizational goals and strive to meet departmental metrics.
• Other duties as assigned.
