Oak Street Health seeks a Clinical Call Center Triage Nurse to extend care beyond clinic hours by addressing patients\' medical, social, and psychological needs over the phone. This remote role supports OSH patients with professional service and clear documentation.

You must hold an active RN license in the US, be able to work remotely with reliable internet, and handle flexible shifts including evenings and weekends.

Qualifications

• Active RN license in all OSH practice states.
• Proactive maintenance of licenses.
• Autonomous time management with reliability.
• High level of conscientiousness and patient focus.
• Willingness to work flexible shifts including evenings/weekends/holidays.

Responsibilities

• Provide confident, professional service to OSH patients.
• Follow current Prescription Refill policy.
• Document clearly during calls and before ending a call.
• Check CareReq queue and ensure completion of requests.
• Schedule appointments and arrange transportation for visits.
• Support organizational goals and meet departmental metrics.
• Other duties as assigned.
