Oak Street Health is seeking a Clinical Call Center Triage Nurse to extend care beyond clinic hours by addressing patients' medical, social, and psychological needs telephonically. You will provide professional service and accurate documentation while supporting remote patients.

This full-time remote role requires a current RN license, flexibility for evenings and weekends, and a commitment to delivering an unmatched patient experience.

Qualifications

• Active, non-probationary RN license in all OSH practice states.
• Strong communication and documentation skills.
• Ability to manage time and work autonomously.

Responsibilities

• Provide confident, professional service to OSH patients on calls.
• Follow current Prescription Refill policy.
• Document accurately during and after each call.
• Schedule appointments and arrange transportation when needed.
• Monitor CareReqs and ensure tasks are completed.
• Support remote patients with transportation coordination for clinic visits.
