Job summary

Oak Street Health is seeking a Clinical Call Center Triage Nurse to extend care beyond regular clinic hours, addressing patients’ medical, social, and psychological needs by phone. You will provide professional service, document thoroughly, and coordinate appointments and transportation for patients.

The role requires an active RN license in all OSH practice states, US work authorization, and the ability to work flexible shifts, including evenings and weekends.

Qualifications

• Active RN license in all OSH practice states.
• US work authorization required.
• Ability to work flexible shifts including evenings, weekends, and holidays.
• Maintain licensure and continue education without prompting.

Responsibilities

• Provide confident and professional service to OSH patients.
• Follow current Prescription Refill policy.
• Document clearly and completely during and after calls.
• Schedule appointments for patients and arrange transportation for clinic visits.
• Maintain CareReqs and ensure tasks are completed within scope.
• SupportCareReq reductions and CET management as assigned.
• Other duties as assigned.
