Job summary

Oak Street Health seeks a Clinical Call Center Triage Nurse to extend care beyond clinics, addressing patients' medical, social and psychological needs by phone. This full-time, remote role requires an active RN license in all OSH practice states and US work authorization.

You will provide professional service, document encounters accurately, schedule appointments, arrange transportation, and contribute to a supportive patient experience.

Qualifications

• Active, non-probationary RN license in all states of OSH practice.
• US work authorization.
• Ability to manage time and work autonomously with reliability.
• Focus on delivering an unmatched patient experience on every call.
• Willingness to work flexible shifts including evenings, weekends, and holidays.

Responsibilities

• Provide confident and professional service to OSH patients.
• Follow current Prescription Refill policy.
• Schedule appointments and arrange transportation as needed.
• Document interactions clearly and completely during calls.
• Complete documentation for all patient requests prior to ending a call.
• Check CareReq queue and ensure CareReqs are complete.
• Complete all tasks within scope and ability of the role.
• Engage with patients without background distraction.
