Job summary

Vail Health seeks a Triage Registered Nurse to support care coordination and triage patient calls using established protocols. You will assess symptoms, document care plans in the EHR, and collaborate with providers to determine the best care path.

The role emphasizes HIPAA/OSHA compliance and contributes to continuous quality improvement in a fast-paced setting. Colorado RN license or valid compact license is required; 3 years of clinical experience preferred.

Qualifications

• 3 years clinical experience with patient care and administrative duties preferred.
• State of Colorado Registered Nurse or valid compact state license required.
• Knowledge of medical terminology, HIPAA compliance and OSHA rules and regulations required.

Responsibilities

• Performs care coordination and nursing evaluations to assess patient needs.
• Triages patient phone calls and portal messages; supports care coordination team.
• Documents symptoms, assessments, and plans in EHR with follow-up actions.
• Participates in weekly huddles to discuss KPI and clinical direction.
• Ensures proper forwarding of pertinent medical information for continuity of care.
• Contributes to quality improvement and CPC+ care coordination initiatives.
