Job summary

Qualifications

• 3 years clinical experience with patient care and admin duties preferred.
• Nursing assessment skills with/without visual inspection.
• Knowledge of medical terminology, HIPAA compliance and OSHA requirements.
• State of Colorado RN or compact state license required.

Responsibilities

• Care coordination and triage to assess severity and disposition.
• Triage patient calls and portal messages.
• Document in EHR with assessment, plan, implementation and evaluation.
• Participate in CPC+ Care Coordination and Performance Improvement activities.
• Join weekly huddles to review KPI and coordinate with providers.
