Job summary

Vail Health seeks a Triage RN to perform care coordination and triage for emergency, urgent care and clinic referrals. You will support providers by triaging calls and portal messages, document in EHR, and participate in KPI reviews and quality improvement.

Minimum requirement is an RN license with state requirements; Colorado or compact license is referenced. Roles may be remote outside Colorado; pay is hourly based on education and experience.

Qualifications

• 3 years clinical experience with patient care and administrative duties preferred.
• Nursing assessment skills with/without visual inspection using basic nursing theory.
• Knowledge of medical terminology, equipment, HIPAA compliance and OSHA rules and regulations.

Responsibilities

• Perform care coordination and professional nursing functions to assess health concerns.
• Triage patient phone calls and portal messages with provider support.
• Document in EHR triage assessment, plan, implementation and evaluation.
• Participate in CPC+ Care Coordination team processes.
• Participate in weekly huddles to review KPI and obtain provider direction as needed.
• Assist in quality improvement activities and ensure continuity of care.
