Qualifications

• 3 years of clinical experience with patient care and admin duties preferred.
• Nursing assessment with/without visual inspection using basic nursing theory.
• Knowledge of medical terminology, equipment, HIPAA and OSHA rules required.

Responsibilities

• Perform care coordination and triage nursing to assess health concerns using protocols.
• Support providers by triaging patient calls and messages.
• Document symptoms, assessment, advice, and patient response in EHR.
• Contribute to KPI reviews and care coordination team meetings.
• Ensure proper forwarding of medical information for continuity of care.
