Oak Street Health is seeking a Clinical Call Center Triage Nurse to extend care beyond clinic hours by addressing patients’ medical, social, and psychological needs via telephone.

Qualifications

• Active, non-probationary RN license in all states where OSH practices.
• Ability to manage time and work autonomously with reliable attendance.
• Willingness to work flexible shifts outside core business hours, including evenings, weekends, or holidays.
• US work authorization required.

Responsibilities

• Provide confident and professional service to OSH patients.
• Follow current Prescription Refill policy.
• Maintain clear and precise documentation during and after calls.
• Complete documentation for all patient requests prior to ending a call.
• Check CareReq queue and ensure requests are complete.
• Schedule appointments for patients as needed and arrange transportation for clinic visits.
