Qualifications

• 3 years clinical experience preferred.

Responsibilities

• Evaluates and assesses patient health care needs using established triage protocols.
• Participates in weekly KPI huddles and ensures KPI-aligned workflows.
• Obtains input from providers to determine the best clinical course of action.
• Documents symptoms, nursing assessment, and patient responses in the EHR.
• Follows policies and protocols to ensure consistency and improve health care outcomes.
• Coordinate and schedule same-day and future appointments on provider schedules.
• Manage prescription refills per protocol.
• Provide patient education on chronic disease prevention and care access.
• Contributes to quality improvement and continuity of care.
