Job summary

You will document assessments in the EHR, follow clinical protocols, and support care coordination initiatives with a focus on quality outcomes and patient education.

Qualifications

• 3 years clinical experience with patient care and administrative duties.
• Knowledge of disease processes and development across age groups.
• Nursing assessment skills with/without visual inspection.
• Knowledge of medical terminology, equipment, HIPAA compliance and OSHA rules.

Responsibilities

• Evaluates and assesses patient care needs using triage protocols.
• Participates in weekly huddles to review KPI and workflow effectiveness.
• Obtains provider input to determine the best clinical actions.
• Documents symptoms, nursing assessments and patient responses in EHR.
• Follows policies to ensure quality care and access.
• Coordinate and schedule same-day and future appointments.
• Manage prescription refills per protocol.
• Provide patient education on chronic disease and prevention.
• Contribute to performance improvement activities.
