Oak Street Health is seeking a Clinical Call Center Triage Nurse to extend care outside regular clinic hours by addressing patients' medical, social, and psychological needs over the phone. This remote role supports OSH patients and requires a current RN license in all OSH practice states.

You'll provide professional service, document calls clearly, follow prescription refill policies, and coordinate appointments and transportation as needed.

Qualifications

• Active, non-probationary state RN license in all OSH practice states.
• Ability to manage time and work autonomously with consistent reliability.
• Willingness to work flexible shifts including evenings, weekends and holidays.
• Proactive maintenance of licenses and ongoing professional development.

Responsabilités

• Provide confident and professional service to OSH patients.
• Follow current Prescription Refill policy.
• Maintain clear and precise documentation during calls.
• Schedule appointments for patients and arrange transportation as needed.
• Check the CareReq queue and ensure CareReqs are complete.
