Job summary

Devoted Health is seeking an Advanced Practice Provider (APRNs or PAs) to lead the Medical Transitions of Care (TOC) program. This role involves virtual care during the 30-day post-discharge window, comprehensive assessments, medication reconciliation, and establishing a whole-person, 30-day care plan.

You will work in a highly interdisciplinary team to reduce readmissions and shape the TOC program. Responsibilities include acute/virtual visits, patient education, and contributing to EHR

Qualifications

• 5+ years of direct patient care managing geriatric populations.
• Active RN/ APRN/ PA license in good standing.
• Experience with telehealth visits.
• Experience in internal medicine, urgent care, emergency room, hospital medicine, post-acute medicine, or geriatrics.
• Ability to work full-time, Mon-Fri, 8:30 AM – 5:00 PM; licenses active and in good standing.

Responsibilities

• Conduct comprehensive post-discharge assessments of medical conditions, medications, functional status, and psychosocial needs.
• Perform post-discharge medication reconciliation and optimize guideline-directed therapy.
• Place orders (prescriptions, labs, imaging, referrals, home health, DME) and provide disease-specific education.
• Perform acute/virtual urgent-care TOC visits and manage symptoms with action bias.
• Adjust plans of care as clinical pictures evolve and coordinate with care teams.
• Participate in interdisciplinary TOC rounds and collaborate with RN case managers, care coordinators, social workers, pharmacy, and PCPs/specialists.
• Educate the broader team and contribute to quality-improvement projects; provide EHR feedback.
• Work within a team to deliver wrap-around care and reduce readmissions.
