Job summary

Devoted Health is seeking an Advanced Practice Provider (APRNs or PAs) to lead our Medical Transitions of Care program. The APP will conduct post-discharge visits within a 30-day window, delivering comprehensive wrap-around care to high-risk geriatric patients via telehealth and in-person encounters.

The role emphasizes acute/urgent symptom management, care coordination, and continuous program development, with a strong interdisciplinary team and a generous compensation package.

Qualifications

• 5+ years of direct patient care with geriatric populations
• Active RN/APRN/PA license in good standing
• Experience with telehealth visits and chronic disease management

Responsibilities

• Conduct thorough post-discharge assessments of medical, medication, functional, and psychosocial needs.
• Perform acute/virtual urgent-care TOC visits and determine appropriate level of care.
• Place orders (prescriptions, labs, referrals) and provide disease-specific education with contingency plans.
• Participate in interdisciplinary TOC rounds and collaborate with care team members.
• Contribute to quality-improvement projects and provide feedback on EHR as the program grows.
• Help build and shape the TOC program and team culture over 30 days.
