Job summary

Devoted Health is seeking an experienced Advanced Practice Provider (APN/PA) to lead our Medical Transitions of Care (TOC) program. This role blends comprehensive initial post-discharge visits with acute/urgent visits within a 30‑day window to reduce readmissions.

You will build a whole-person, chronic-care plan, collaborate with a multi-disciplinary team, and help shape the EHR while growing the TOC program. The role supports full-time, remote work across states with licensing requirements.

Qualifications

• Experience caring for elderly patients with multiple chronic conditions in acute and post-acute settings.
• Ability to navigate both acute management and longitudinal chronic care.
• Comfort delivering care via telehealth platforms and coordinating with interdisciplinary teams.

Responsibilities

• Perform comprehensive initial TOC visits with post-discharge assessments and medication reconciliation.
• Conduct acute/virtual urgent-care TOC visits and determine level of care and necessary interventions.
• Manage the 30-day care trajectory, adjusting plans as clinical conditions evolve.
• Participate in interdisciplinary TOC rounds and collaborate with care coordinators, social workers, pharmacy, and PCPs.
• Support build-out of the TOC program, contribute to quality-improvement projects, and provide EHR feedback.
• Adapt to evolving service needs and participate in organization-wide initiatives.
