Job summary

Devoted Health in the United States is seeking an experienced Nurse Practitioner/Physician Assistant to lead the Transitions of Care (TOC) APP program. This fully remote role reports to the Medical Director and focuses on clinical excellence, team empowerment, and process improvements for post-discharge patients.

You will supervise APPs, drive KPIs, oversee scheduling and onboarding, collaborate with program leadership, and advance TOC workflows while providing direct patient care as needed.

Qualifications

• 5+ years working in clinical practice as an Advanced Practice Provider (NP/PA).
• At least 1 year of experience seeing post-discharge patients as an Advanced Practice Provider (NP/PA).
• 2+ years of previous management or team leadership experience.
• Active RN and APRN license in home state with national APRN certification and/or multi-state licensing ability.
• Must be able to work Monday–Friday 9:30am–6:00pm EST.

Responsibilities

• Oversee clinical performance of the APPs in the TOC program.
• Provide TOC patient care 40% of the time and manage direct reports.
• Develop, track, and manage KPIs for TOC APPs.
• Lead recruitment, onboarding, and training of TOC APPs.
• Collaborate with program leadership to improve TOC workflows.
