Devoted Health is hiring an Advanced Practice Provider (Nurse Practitioner or Physician Assistant) for the Medical Transitions of Care program. The role combines acute and longitudinal care for high-risk patients during a 30‑day post-discharge window, delivering comprehensive, wrap-around care via virtual and in-person visits.

The APP will help build the TOC program, contribute to quality-improvement, and collaborate with inter-disciplinary teams to prevent readmissions and optimize patient

Qualifications

• 5+ years of direct patient care with geriatric, multimorbidity populations.
• Active RN/ARNP/PA licensure in current or future Devoted states, willing to license in 90 days.

Responsibilities

• Conduct comprehensive initial TOC visits post-discharge and perform medication reconciliation and orders.
• Perform acute/virtual urgent-care TOC visits and determine appropriate level of care.
• Manage patient plan across 30 days with adjustments for evolving clinical status.
