Job summary

Devoted Health is seeking an experienced Advanced Practice Provider (APRNs or PAs) to build and lead our Medical Transitions of Care program. The role blends comprehensive initial post‑discharge visits with ongoing follow‑up in a high‑risk period, using a collaborative, interdisciplinary approach.

You will shape the TOC program, deliver virtual and in‑person care, and contribute to quality‑improvement work and EHR enhancements. Florida‑based, full‑time, with a patient‑centered focus.

Qualifications

• Minimum 5+ years of direct patient care with geriatric, chronic conditions.
• Licensed APRN or PA with eligibility in Devoted Health states.
• Experience delivering post‑discharge and telehealth visits.

Responsibilities

• Conduct comprehensive initial TOC visits and post‑discharge assessments.
• Perform acute/virtual urgent‑care TOC visits with clear escalation when needed.
• Manage 30‑day care trajectory and adjust plans as conditions evolve.
• Participate in interdisciplinary rounds and coordinate with care teams.
• Support program development and contribute to EHR improvements.
