Job summary

Devoted Health is seeking experienced APPs (APRNs/PAs) to lead the TOC program, delivering post-discharge care virtually to high-risk geriatric patients. Expect comprehensive initial visits and follow-ups, with an emphasis on decompensation recognition, plan development, and team collaboration.

Role includes program-building duties, quality-improvement work, and shaping a safe, open communication culture while using a homegrown EHR. Telehealth experience and active licenses are required.

Qualifications

• 5+ years of direct patient care with geriatric populations.
• Experience performing telehealth visits and managing acute/chronic disease.
• Active RN/APRN/PA licensure in good standing.

Responsibilities

• Conduct comprehensive post-discharge assessments of medical, medication, functional, and psychosocial needs.
• Perform post-discharge medication reconciliation and optimize therapy.
• Place orders (prescriptions, labs, imaging, referrals, home health, DME) and provide patient education.
• Perform acute/virtual urgent-care TOC visits and determine level of care as needed.
• Adjust care plans in response to evolving clinical picture and patient priorities.
• Participate in interdisciplinary TOC rounds and contribute to QI efforts and EHR feedback.
