Devoted Health is seeking an Advanced Practice Provider (APRNs or PAs) for our Medical Transitions of Care (TOC) program. You will deliver virtual and in-person care to high‑risk, recently discharged patients within a 30‑day window with a focus on preventing readmissions.

The role blends comprehensive initial TOC visits (70%) with follow‑ups and acute visits (30%), requiring strong clinical judgment and hands-on chronic disease management across a care continuum.

Qualifications

• 5+ years of direct patient care with geriatric populations and multiple chronic conditions.
• Understanding of acute and longitudinal chronic disease management and their integration.
• Ability to work full-time Monday–Friday, 8:30 AM–5:00 PM (local time).
• RN and APRN or PA licenses active and in good standing; eligible to license in required states within 90 days.

Responsabilités

• Perform comprehensive initial TOC visits including post-discharge assessments and medication reconciliation.
• Conduct acute/virtual urgent-care TOC visits and determine level of care with a bias to action.
• Manage patient plans across the 30-day window, adjusting care as clinical needs evolve.
• Participate in interdisciplinary TOC rounds with RNs, care coordinators, social workers, and physicians.
• Foster safe, open communication and contribute to quality-improvement projects and EHR enhancements.
• Support expansion of services and adapt to a fast-paced, changing environment.
