Job summary

Humana Inc. is seeking a Care Manager, Telephonic Behavioral Health 2 to assess and support members’ wellness through telephonic care coordination. Duties include evaluating needs, guiding to resources, and facilitating engagement with care plans.

The role involves independent judgment within guidelines, with travel minimal and a remote work setup. The ideal candidate will hold a Master’s in behavioral health, a relevant license (LMSW/LCSW/LPC/LMHC), and 2+ years of behavioral health experience,

Qualifications

Current valid social service license (LMSW/LCSW/LPC/LMHC).
Master's degree in behavioral-health field from an accredited university.
2+ years of experience as a behavioral health professional.
Experience with behavioral change, health promotion, and coaching.
Proficiency with Microsoft Office programs (Word, Excel, PowerPoint).

Responsibilities

Assess and evaluate members' needs to achieve and/or maintain optimal wellness.
Guide members toward and facilitate interaction with resources for care and wellbeing.
May create member care plans and monitor progress toward outcomes.
Identify and resolve barriers to effective care; work with telephonic team and stakeholders.
