Qualifications

• Current valid social service license, e.g. LMSW, LCSW, LPC, LMHC.
• Master's degree in a behavioral-health field from an accredited university.
• 2+ years of experience as a behavioral health professional.
• Experience with behavioral change, health promotion, and coaching.
• Proficiency with Microsoft Office Programs (Word, Excel, PowerPoint) and shared systems.

Responsibilities

• Assess and evaluate members' needs to achieve and maintain optimal wellness.
• Guide members/families toward resources and facilitate interaction with services.
• Monitor progress and adjust care plans to meet outcomes.
