Job summary

Humana Inc. seeks a Care Manager, Telephonic Behavioral Health 2, to assess members’ needs and guide families toward appropriate behavioral health resources in a telephonic setting.

You will create and monitor care plans, resolve barriers, and support progress toward optimal wellness with minimal supervision. This remote role requires licensure in behavioral health, a master’s degree, and 2+ years of experience, with proficiency in MS Office and telephonic patient interactions.

Qualifications

• Valid social service license (LMSW/LCSW/LPC/LMHC) required.
• Master's degree in behavioral-health field from an accredited university.
• 2+ years of experience as a behavioral health professional.
• Experience with behavioral change, health promotion, and coaching.
• Proficiency with Microsoft Office programs (Word, Excel, PowerPoint).

Responsibilities

• Assess and evaluate members' needs and requirements to guide care.
• Identify and resolve barriers to effective care and progress toward outcomes.
• Create and monitor member care plans and coordinate resources.
