Job summary

Humana is seeking a Field Care Manager, Behavioral Health, to conduct face-to-face and telephonic assessments, coordinating behavioral health and medical services across the interdisciplinary team.

You’ll engage with diverse members, including those with SMI/SED, to ensure timely, integrated care and improved health literacy while managing care transitions. This remote role requires travel up to 25% to meet with members and collaborate with providers.

Qualifications

• Bachelor’s degree in social work, psychology or related field.
• Virginia licensed LMHP, LPC or LCSW.
• Minimum of 2 years of post-degree clinical experience in behavioral health.
• Case management experience with complex SMI or SED.
• Must reside in the Charlottesville region of Virginia (Lynchburg or Fishersville).
• Ability to travel to region-based facilities and homes for face-to-face assessments.
• Proficient with electronic medical records and MS Office.
• Full-time schedule; valid driver's license and reliable transportation.

Responsibilities

• Perform face-to-face and telephonic assessments.
• Coordinate behavioral health and medical services to members.
• Serve as quarterback of the ICT and lead ICT meetings.
• Improve member health literacy and address health-related social needs.
• Collaborate with providers and community organizations for seamless care.
• Ensure compliance with DMAS, CMS and NCQA requirements.
