Qualifications

• Current valid social service license (e.g., LMSW, LCSW, LPC, LMHC).
• Master's degree in a behavioral-health field from an accredited university.
• 2+ years or more of experience working as a behavioral health professional.
• Experience with behavioral change, health promotion and coaching.
• Proficiency with Microsoft Office programs (Word, Excel, PowerPoint) and shared systems.

Responsibilities

• Assess and evaluate members' needs to achieve and maintain optimal wellness.
• Guide members/families toward resources appropriate for care and wellbeing.
• Monitor patient care and progress; create or adjust member care plans.
• Make independent decisions within guidelines and follow procedures.
