Job summary

Humana Inc. invites applications for a remote Telephonic Behavioral Health Care Manager 2. The role assesses members’ needs, guides families toward resources, and may create care plans while monitoring progress toward wellness goals.

The position requires a behavioral health license, a Master’s in a related field, and 2+ years of experience, with weekly 40 hours and a pay range of $59,300–$80,900. Eligible for a bonus plan and comprehensive benefits are described.

Qualifications

• Current valid social service license (LMSW, LCSW, LPC, LMHC).
• Master's degree in behavioral-health field from an accredited university.
• 2+ years of experience as a behavioral health professional.
• Experience with behavioral change, health promotion, and coaching.
• Proficiency with Microsoft Office Programs (Word, Excel & PowerPoint) and shared systems.

Responsibilities

• Assess and evaluate members' needs and guide them toward appropriate care resources.
• Identify barriers to care and facilitate care plans; monitor progress and outcomes.
