Job summary

Qualifications

• Licensed social work/counseling license (e.g., LMSW/LCSW/LPC/LMHC)
• Master's degree in a behavioral-health field from an accredited university
• At least 2 years of experience as a behavioral health professional

Responsibilities

• Assess and evaluate members' needs to guide toward resources for care and wellbeing
• Identify barriers to care and monitor progress toward outcomes
• Create and manage member care plans when appropriate
