Qualifications

• Must hold a current valid social service license (e.g., LMSW, LCSW, LPC, LMHC).
• Master's degree in a behavioral-health field from an accredited university.
• 2+ years of experience as a behavioral health professional.
• Experience with behavioral change, health promotion, and coaching.
• Proficiency with Microsoft Office Programs (Word, Excel, PowerPoint).

Responsibilities

• Assess and evaluate members' needs to achieve or maintain wellness.
• Identify and resolve barriers to care and guide toward resources.
• Monitor progress and adjust care plans as needed.
• May create member care plans and ensure HIPAA-compliant information handling.
