Fuze Health is seeking a Registered Nurse to support the telehealth care team by providing remote outreach to Medicare members and ensuring compliance with Stars measures. This role focuses on high-quality virtual care and accurate documentation.

The candidate should have a BSN, an active nursing license, 2+ years of RN experience, and 1–2 years in a remote/telehealth setting, with strong EMR skills and customer service abilities.

Qualifikationen

• BSN degree and active, non-restricted state license.
• Minimum 2 years of RN experience.
• 1–2 years in remote or telehealth settings preferred.
• Strong computer skills and EMR familiarity.
• No visa sponsorship available at this time.

Aufgaben

• Telehealth Patient Care: provide remote patient care and support via telehealth platforms.
• Virtual Assessments: conduct assessments, review histories and symptoms.
• Medical Documentation: accurately document patient conditions and care interactions.
• Educate patients and families on conditions, meds and self-care.
• Care Plan Collaboration: work with providers to implement individualized plans.
• Regulatory Compliance: maintain records per healthcare regulations and privacy laws.
• Continuous Education: stay updated on telehealth protocols and best practices.
