Job summary

Fuze Health is seeking a Registered Nurse to join its telehealth care team. This remote role focuses on Medicare member outreach, ensuring Stars measure compliance and ED discharge follow-up. You will perform virtual assessments, document patient data, and educate families while collaborating with providers.

The position offers a Monday–Friday schedule with one Saturday per month, remote work, and comprehensive benefits. Prior telehealth experience is preferred.

Qualifications

• BSN degree and active, non-restricted license in nursing.
• 2+ years of nursing experience with remote or telehealth settings preferred.
• Strong computer skills and EMR familiarity; excellent customer service.
• Visa sponsorship not available at this time.

Responsibilities

• Provide remote telehealth patient care and support to Medicare members.
• Conduct virtual assessments, review histories and symptoms.
• Document patient conditions accurately and maintain care records.
• Educate patients and families on conditions, medications and self-care.
• Collaborate with care teams to develop and implement plans.
• Ensure regulatory compliance and privacy protection in all duties.
• Engage in ongoing education on telehealth and clinical practices.
