Fuze Health is seeking a Registered Nurse to support the telehealth care team, performing remote outreach to Medicare members and ensuring compliance with Stars measures and ED discharge follow-ups.

The role requires a BSN, active RN license, and 2+ years of nursing experience, including 1–2 years in telehealth. Strong EMR skills and a commitment to compassionate virtual care are essential.

Qualifications

• BSN degree required.
• Active, non-restricted RN license with primary state licensure in one of the listed states.
• 2+ years of RN experience.
• 1-2 years of remote/telehealth experience.
• Strong computer skills and EMR familiarity.
• Clear verbal and written communication.
• Ability to follow SOPs in fast-paced settings.
• Strong customer service and interpersonal skills.

Responsibilities

• Provide remote patient care via telehealth platforms.
• Conduct virtual patient assessments and review medical histories.
• Document patient conditions, histories, and interactions.
• Educate patients and families on conditions, medications, and self-care.
• Collaborate with healthcare providers to develop care plans.
• Maintain regulatory compliance and privacy standards.
• Engage in ongoing telehealth education and best practices.
• Communicate with empathy and professionalism in all patient interactions.
