Job summary

Fuze Health Inc. is seeking a Registered Nurse to support the telehealth care team, providing remote outreach to Medicare members and ensuring compliance with a new Stars measure and safe ED discharge.

Responsibilities include telehealth visits, virtual assessments, documentation, patient education, and collaboration with care teams. Must hold a BSN and an active RN license; visa sponsorship is not available.

Qualifications

BSN degree and active RN license
2+ years of RN experience
~1–2 years remote telehealth experience
Strong computer skills and EMR familiarity
Strong customer service and interpersonal skills
Ability to work independently and in a collaborative care team
Visa sponsorship not available

Responsibilities

Telehealth Patient Care: Provide remote patient care and support through telehealth platforms.
Virtual Assessments: Conduct virtual patient assessments, including reviewing medical histories and current symptoms.
Medical Documentation: Accurately assess and document patient conditions, histories, and care interactions.
Educate patients and their families on medical conditions, medication adherence, and self-care practices.
Care Plan Collaboration: Work closely with other healthcare providers to develop and implement individualized care plans.
Regulatory Compliance: Maintain accurate records and ensure adherence to healthcare regulations and privacy laws.
Continuous Education: Stay up to date with medical advancements, telehealth protocols, and best practices through continuing education.
