Job summary

Optum Home & Community Care is seeking a National On-Call NP to provide telephonic/telehealth care 24/7 from a remote US location. The role covers evenings, nights, weekends and holidays, with a required 40-hour weekly commitment.

You will use EMR, coordinate care across facilities and providers, document to CMS standards, and participate in ongoing training. This position offers a $10,000 sign-on bonus for external candidates and a comprehensive benefits package.

Qualifications

• NP: Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program.
• Active and unrestricted license in the state of Colorado and ability to start the process of obtaining additional licensure prior to start date.
• Current DEA licensure/prescriptive authority or ability to obtain post-hire.
• 3+ years of clinical experience as an APC.
• Proficient computer skills including EMR documentation.

Responsibilities

• Available on telephonic platform to coordinate and manage care for members between caregivers, facilities, hospitals, PCPs and Optum staff.
• Utilize EMR to provide acute care during shifts and holiday hours.
• Deliver cost-effective, quality care to members and manage chronic, acute conditions with collaboration of physicians.
• Coordinate care as members transition through different levels of care and settings.
• Attend mandatory educational and MyLearning training requirements.
• Participate in quality initiatives and ensure care plan aligns with CMS regulations.
