The role supports patients, caregivers, and facilities, coordinating with physicians and the care team while using EMR for documentation.

Qualifications

• NP: Master of Science in Nursing or Doctor of Nursing Practice.
• Active and unrestricted NP license in the state of practice and ability to obtain in other required locations.
• Proficient in electronic medical records (EMR).
• Ability to work scheduled shifts and meet scheduling policies.

Responsabilités

• Provide telephonic/telehealth care to patients and caregivers.
• Coordinate care across providers, facilities, and the Optum team.
• Utilize EMR to document assessments and care plans.
• Deliver cost-effective, quality care for ambulatory patients.
• Attend mandatory trainings and maintain privacy standards.
