Job summary

UnitedHealth Group’s Optum Home & Community Care is seeking a Telehealth National On-Call APC (Nurse Practitioner) to deliver telephonic/telehealth care from a remote home-based setting. The role requires 20 hours per week with evening, night, and weekend coverage across markets in the U.S.

Applicants must hold an active PA NP license in Pennsylvania and be able to work within a multi-market, interdisciplinary team. A sign-on bonus up to $20,000 is available for external candidates.

Qualifications

• 3+ years of clinical experience as an APC
• Active and unrestricted PA NP license
• Ability to obtain collaborative practice agreement if applicable

Responsibilities

• Provide telephonic/telehealth care to members 24/7
• Utilize EMR to provide acute care and document encounters
• Coordinate care across care settings and with providers
• Participate in quality initiatives and ensure compliant documentation
