Job summary

Optum Home & Community Care seeks a field-based Nurse Practitioner to perform annual in-home health assessments for Medicare Advantage members. The role emphasizes preventive care, education, and linking patients to needed resources while enabling autonomy and meaningful patient impact.

Offers a $10,000 sign-on bonus or $20,000 student loan repayment, flexible 8 AM–8 PM scheduling, 20–40 hours per week, and no on-call, weekends, or holidays.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active, unrestricted Nurse Practitioner license and ability to obtain licensure before start date.
• Active, unrestricted driver's license and reliable transportation for home visits.
• Ability to perform assessments in varied home environments and meet physical demands of home-based care.

Responsibilities

• Conduct comprehensive in-home health assessments for Medicare Advantage and other members.
• Collaborate with primary care providers to close care gaps and address social determinants of health.
• Educate members and connect them to resources to support staying healthy at home.
