Job summary

Optum Home & Community Care seeks a Telehealth Urgent Care APC for a remote Part-Time 20-hour per week role, serving patients across the United States. The role requires evenings, nights and rotating weekend coverage, plus holidays.

You will deliver telephonic/telehealth care, coordinate with caregivers and facilities, and utilize EMR to document care while ensuring CMS-compliant records. This position offers sign-on incentives and a work-from-home arrangement.

Qualifications

• 3+ years of clinical APC experience.
• Active CT or RI NP license required.
• Ability to gain autonomous practice where allowed by state law.
• Graduate of MSN or DNP program.
• Board certification: FNP/ANP/Gerontology/ACNP required or eligible.

Responsibilities

• Provide telephonic/telehealth care to patients, caregivers and facility staff 24/7.
• Utilize EMR to document and manage care across shifts including holidays.
• Coordinate care transitions and communicate with multidisciplinary teams.
• Deliver cost-effective, high-quality care across medical and behavioral health needs.
• Attend mandatory trainings and ensure compliance with CMS documentation.
