Job summary

Optum Home & Community Care is seeking a Telehealth Urgent Care APC to provide telephonic/telehealth care for members across the United States. The role requires an active CT or RI NP license and board certification in NP specialties, with a DEA registration or ability to obtain post-hire.

This Part-Time (20 hours) remote position offers a sign-on bonus up to $20,000 for external candidates and requires rotating evenings/nights and weekends.

Qualifications

• Graduate of an accredited MSN or DNP program.
• Active NP license in CT or RI; ability to obtain in other locations.
• Board certification by AANP or ANCC in appropriate NP track (Family/Adult/Gerontology).
• Current DEA licensure/prescriptive authority or ability to obtain post-hire.
• Ability to work scheduled shifts and maintain patient privacy in a virtual setting.

Responsibilities

• Provide telephonic/telehealth care and direction to members, caregivers and staff 24/7.
• Utilize EMR to document and manage care; assess and treat acute/ chronic conditions.
• Coordinate care across facilities, hospitals and PCPs; adjust plans as needed.
• Deliver high-quality, cost-effective care and participate in quality initiatives.
