Optum Home & Community Care is seeking a part-time Advanced Practice Clinician (Nurse Practitioner) to provide telephonic/telehealth care from a remote location in the U.S. The role requires 20 hours per week with weekend and holiday coverage on a rotating basis.

Active PA NP license is required with ability to obtain in other locations. A sign-on bonus up to $20,000 is available for external candidates, with compensation ranging from $109,500 to $164,000 annually.

Qualifications

• 3+ years of clinical APC experience.
• Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program.
• Active and unrestricted Nurse Practitioner license in Pennsylvania.

Responsabilités

• Provide telephonic/telehealth care and direction to patients, caregivers and staff on a 24/7 basis.
• Deliver telehealth care using provided telephonic platform; may use video as needed.
• Utilize EMR to document care and manage encounters across shifts and holidays.
• Coordinate care during transitions between levels of care and settings.
• Participate in quality initiatives and maintain required trainings.
