Qualifications

• Active PA NP license with ability to practice in Pennsylvania.
• MSN or DNP with APRN specialization.
• Board-certified/eligible Nurse Practitioner in relevant scope.

Responsibilities

• Provide telephonic/telehealth care for 24/7 coverage including holidays.
• Deliver care and coordinate with caregivers, facilities, hospitals and PCPs.
• Document encounters accurately per CMS and regulatory requirements.
• Utilize EMR to manage acute and chronic conditions during all shifts.
